Defibrillator Resume CPR and ACLS measures until alternative defibrillator is brought to the bedside. Every department should know where the closest “BACK-UP” is located (i.e., down the hall, next room, etc.)
Monitor 1) Retrieve a transport monitor and connect the patient
    OR
2) Retrieve defibrillator. The monitor portion can be connected to patient for monitoring purposes.
After Clinical Engineering hours, notify the Nursing Supervisor. Clinical Engineering will either respond by phone or Nursing Supervisor will know the location of Clinical Engineering’s spare roll-around monitor.
Ventilator Hand-ventilate patient until spare ventilator arrives. Notify the Respirator Therapist assigned to the specific unit or page the Therapist-in-Charge (719-4946) to obtain spare ventilator.
Dialysis Machine Remove blood-line from venous line clamp. Then manually hand-crank blood-pump to return patient’s blood. After hours, obtain another dialysis machine. The dialysis technician will investigate the malfunctioning unit the following work day.
Heart-Lung Machine and/or ECMO Pump Electrical Failure
Utilize hand crank.

Mechanical Failure (crank does not work)
Remove tubing from the malfunctioning pump head and place in other pump.

Heart-Lung Machines”  Replace malfunctioning pump with spare pump (located in pump room). Notify Clinical Engineering OR Shop (353-1736)



ECMO:  Change out malfunctioning equipment with spare.  Contact Clinical Engineering main shop. (353-1727)
Anesthesia Unit Ventilator
Hand-bag patient until spare unit arrives.

Gas Wall-Outlet Failure
Connect patient to the E-tank (located on cart).
Obtain spare unit.  Notify Clinical Engineering (Parnassus 353-1736) or (Mount Zion 885-7642).

Notify Medical Center Facilities (Parnassus –3-1120) or
(Mount Zion – 5- 7576).
Ventricular Assist Device (VAD) Thoratec
Utilize hand bulbs in back of machine until spare arrives.

BioMedicus
Utilize hand crank (2 cranks) until spare arrives.
The Medical Center has a total of three (3) machines (2 Thoratecs and 1 Bio-Medicus), all located in the Operating Room. Notify Clinical Engineering OR Shop (353-1736).
Continuous Veno/Venous Hemofiltration (CVVH) Utilize hand crank to prevent blood from clotting until spare pump arrives Contact Central Services 353-1837 to obtain a replacement pump.

Obtain replacement pump immediately. The Medical Center has three (3) IAPBs (two located in Intensive Cardiac Care and one in the Operating Room).
Emergency Aspirator Hand bag patient until spare unit arrives. Contact Central Service (Parnassus 353-1837) or (Mount Zion 885-7255) to request an emergency replacement aspirator.
Pumps, Infusion or PCA Stop pump. Contact Central Services (Parnassus 353-1837) or Mount Zion (885-7255) to request an emergency replacement pump.
ALL other equipment Stop using equipment and attain spare equipment from appropriate source. If after hours and device needs immediate repair, notify Nursing Supervisor in order to get a Clinical Engineer to respond to problem.
